sharing of STD case follow-up information to any party outside the public health

This site uses a secure server for conducting online transactions. All security
information that you transmit to us will be protected by 128-bit encryption

notifications or referrals will be successful and that the successful outcome of
the follow-up and referral may be contingent upon my continued cooperation to
provide information and assistance to the Disease Intervention Specialist
assigned to work with me throughout this process.
